Safety 7 - Session 5

Safety 7 - Session 5 explains the important role of investigations as a result of injuries, minor injuries and property damage to identify root causes and underlying contributors to events with the purpose of learning from our mistakes and prevent additional events.  The user will learn how to use a root cause analysis, a fishbone diagram, and the 5 whys to identify and remedy all contributors to injuries and incidents.
